A crown, in my mind, is needed when more than half of the biting surface of the tooth has been restored or filled. We know, science shows. That that tooth is less than half of its original strength. Even if the tooth's not broken, I can predict that tooth is at a higher risk for breaking. In that case I would explain the risk to the patient, explain that without a crown, this tooth may break, and at that point we can still probably fix it, but the prognosis isn't going to be as good because there's less tooth to work with. There's also a possibility if it breaks the wrong way, tooth would need a root canal. Maybe you could even lose the tooth. The proactive treatment is less costly over the long run than needing a root canal or replacing the tooth. If the tooth needs to be replaced, if a tooth's already been lost, we would talk about an implant or a bridge or a partial denture.
